After creating the departments, please assign the operators to those departments. .

This can be done by

System configuration -> Assignments

All depts will be listed (having their own small box) and all operators will be listen in each dept's box.  Now check the operator's check box AND press "edit" button and the operator is now assigned to that dept.

Now you can "Launch Request Monitor" and it'll hopefully work.
