Title: Pre chat survey / Post chat survey

Looking to customize the pre chat survey and add a post chat survey

Basically.  .  .    before entering chat asking
Name / Order ID / email address / general query 1 / general query 2

after chatting is done offer the opportunity to fill out a questionare
basically adding functions or options to the current survey
how was the help you receved (ratio buttons)
were all your questions answered (drop down)
any comments for us (textarea)
would you like this chat emailed to you (checkbox) - then email it to the customer

prechat stored with the rest of the data and viewable when reviewing chat
post chat stored as well for review of chat operator performance (with link to chat)
